ICSI (INTRACYTOPLASMIC SPERM INJECTION) TREAMENT

What is ICSI : Intracytoplasmic sperm injection is a micro manipulation technique used in the process of fertilization (IVF). It involves injecting a single sperm into the center of a mature oocyte under a microscope (i.e. injecting sperms through the egg’s shell so that sperm does not need to penetrate the shell in order to fertilize the egg).

ICSI is also known as Micromanipulation and involves sophisticated and advanced techniques and equipments. This technique has changed the approach towards male infertility and has given hopes to male patient who has low sperm or Nil sperm count.

In this procedure one sperm is injected directly into each mature egg under a specialized inverted microscope with microinjection pipette. The reported pregnancy rate in an ICSI procedure is same as in IVF – ET alone.

DIFFERENCE BETWEEN IVF & ICSI

  • In IVF fertilization takes place in a dish where many sperm are placed near an egg.
  • Whereas in ICSI an embryologist selects a single sperm to be injected directly into an egg.

Intracytoplasmic sperm injection (ICSI), is an in vitro fertilization (IVF) procedure in which a single sperm is injected directly into an egg. Although certain severe forms of male infertility have a genetic origin, others may be the result of environmental factors. During the past decade, ICSI has been applied increasingly around the world to alleviate problems of severe male infertility in human patients who either could not be assisted by conventional IVF procedures or could not be accepted for IVF because too few motile and morphologically normal sperm were present in the ejaculate of the male partner

HOW IS ICSI PERFORMED/ STAGES OF ICSI

  • Couple counseling: Couples should be aware of all pros and cons of ICSI. Couples should be provided each and every information before aheading to ICSI treatment.
  • Collection of EGG/OVUM pickup
  • Once mature egg follicles have formed the eggs are retrieved under GA through ultrasound guidance & fluid is handed over to the embryologist.
  • Sperm Collection: Fresh sperm is collected Frozen semen through TESA or PESA can be taken.
  • Sperm Washing: During sperm washing process sperm is separated from seminal fluid.
  • ICSI: A holding pipette stabilizes the mature oocyte with gentle suction applied by a microinjection.
    Very sharp & hollow needle is used to immobilize & pick up a single sperm.
    The oocyte is pierced through the cell membrane & the sperm is injected in the cytoplasm of the egg & the needle is removed.
    After the procedure the oocyte will be placed into cell culture & checked on the followingday for signs of fertilization.

WHO NEEDS ICSI?

Couple who are suffering or diagnosed with following infertility issues may require ICSI such as:

  • Low number of sperm formation or ejaculation.
  • Abnormal form of sperms
  • Problems with fragmentation of sperms DNA
  • Cancer of testicular region.
  • Frozen sperm due to availability of husband at the time of egg retrieval procedure.
  • Unexplained infertility: In which all reports of both husband & wife are normal but in spite of that they are unable to have a child.

ICSI SUCCESS RATE

Intracytoplasmic sperm Injection (ICSI) treatment is a process where sperm is directly injected in the egg and then fertilization occurs. After fertilization the embryo is transferred in the uterus for further growth. This process is used if the male partner is suffering from severe infertility.

Who are recommended for ICSI Process?

    • If couples have unexplained infertility
    • Multiple IVF failures.
    • Male partner suffering from Ejaculatory failure, azoospermia, sperm motility
    • Men who went under vasectomy
    • If men are suffering from disease like HIV, hepatitis etc.
    • Men have frozen sperm before undergoing chemotherapy or radiotherapy

ICSI success rate is similar to IVF process. ICSI process is suitable for the patients, who has male infertility so it is different for the partner otherwise, for male partner it is similar to IVF process.
